Instructions for completing the Health Form

All entering students must submit completed health forms prior to registration. The health form consists of three parts:

Part I: Health History is completed by the student.
Part II: Physical Examination is completed by a physician.
Part III: Immunization Record requires additional documentation.

The student alone is responsible for insuring that all these forms are complete and current. A copy of all material including updates must be kept by each student at all times. Students should provide updates, additions, and/or changes as they occur or as requested.
