How to Create an IT Incident

  • Click on “Create a New Information Technology Incident”.

  • Fill in required fields as defined below.

  1. Enter your phone number or “None”.
  2. Enter your University or alternate email address.
  3. Click “Incident Area” and select the appropriate category.
  4. Provide an explanation of your problem.
  5. Click “Submit”.

Note: After a request is submitted an email will be sent to your University account with a ticket number. Please save this ticket number for future reference.
