Prior to Arrival

Essential Documents

The following items must be returned to the Admission Office prior to arrival on campus:

  1. Deposit
  2. Housing form
  3. Acknowledgement of Admission form
  4. One passport sized color photo (size 1 3/8 by 1 3⁄4 inch) print name (and social security number, if applicable) on the back of the photo. Photo is required for University ID cards. Smaller or larger photos will not be accepted. If you prefer to have your application photo scanned for this purpose, please inform your admission officer immediately.
  5. Student Health Insurance confirmation form, including medical air evacuation. Please submit to the Student Health Insurance Office.
  6. Health form – Part I, II and III: Please have this form completed by your physician as soon as possible and return to our office.

PLEASE NOTE: YOU WILL NOT BE PERMITTED TO REGISTER IF WE DO NOT HAVE A COMPLETED HEALTH HISTORY REPORT ON FILE FOR YOU.

Financial Aid Package:

All required paper work for loans and/or scholarships must be completed and returned as soon as possible to assure proper allocation of funds. If you have outstanding financial aid issues, they must be cleared prior to arrival on campus.

Follow the payment schedule according to monies due until loans are approved.

The Office of Student Finances and Financial Aid Services are two separate offices and perform two distinct functions.

Missing admission documentation must be submitted prior to registration.

Registration voucher:

You should receive a registration voucher prior to arriving on campus. You will not receive a voucher if your finances are not in order or if you are missing admission documents. If you have not received a registration voucher please contact the appropriate department, either Student Finances or Admission, to address any unresolved issues before arriving on campus.
